If you would like your name to be included on a list of individuals that have received fraudulent checks purporting to be from this institution, please send an e-mail to ehealth@uwhealth.org and provide us with your: (1) name; (2) check number; (3) city, state; and (4) phone number. This fraudulent activity has been reported to the Federal Bureau of Investigation, and your contact information will be passed along to the FBI should they need it in connection with their investigation of this matter. Until further notice, keep whatever documentation you might have relating to the fraud for at least 60 days. Someone from UW Hospital and Clinics or the FBI will contact you within that time frame if anything further is needed from yourself. (In other words, if you don't hear from us in 60 days, you do not need to call back, and you should then handle the documents according to your own document retention policies.)
