COUNT 1: ALANA NICOLE MANKO between November 15, 2015 and August 22, 2016, in Palm Beach County Florida, did unlawfully agree, conspire, combine or confederate with another person or persons including but not limited to THOMAS STANLEY AND/OR JESSE PETERS AND/OR STEVEN, MANKO AND/OR IRENE MANKO to unlawfully solicit or receive from COASTAL LABORATORY LLC a commission, bonus, kickback, or bribe, directly or indirectly, in cash or in kind, or engage in any split-fee arrangement, in any form whatsoever, in return for referring patients or patronage to COASTAL LABORATORY LLC contrary to Florida Statute 777.04(3) and 817.505(1)(b), (4). COUNT 2: ALANA NICOLE MANKO on or about April 13, 2016, in Palm Beach County Florida, did unlawfully solicit or receive from COASTAL LABORATORY LLC a commission, bonus, kickback, or bribe, directly or indirectly, in cash or in kind, or did engage in any split-fee arrangement, in any form whatsoever, in return for referring patients or patronage to COASTAL LABORATORY LLC contrary to Florida Statute 817.505(1)(b) and (4) (777.011). COUNT 3: ALANA NICOLE MANKO on or about April 22, 2016, in Palm Beach County Florida, did unlawfully solicit or receive from COASTAL LABORATORY LLC a commission, bonus, kickback, or bribe, directly or indirectly, in cash or in kind, or did engage in any split-fee arrangement, in any form whatsoever, in return for referring patients or patronage to COASTAL LABORATORY LLC contrary to Florida Statute 817.505(1)(b) and (4) (777.011).
