Indiana State Department of Health v. Bethesda Gardens
The Indiana State Department of Health and Bethesda Gardens have entered into a Consent Decree. The Consent Decree orders that Bethesda Gardens is permanently rejoined from providing services outside the scope of practice for unlicensed assisted living facilities. In addition, Bethesda Gardens agrees that ISDH will have continuing oversight. Bethesda Gardens agrees to cease operations of is Memory Care Unity by March 31, 2019. Additionally, Bethesda Gardens agrees to future surveys of the facility and to discontinue providing the following services to residents: Full assist bathing, medication administration and centralized storage, and oxygen tank flow control.
