On April 7, 2014 the Illinois Department of Insurance issued a voluntary license revocation for this company, stating in a disciplinary report:
"Insurance producer license and business entity license voluntarily revoked effective April 7, 2014. Mr. Kaczkowski had been licensed to sell casualty insurance since 1991 and ZK Insurance Group had been licensed since 2002. Mr. Kaczkowski accepted the Voluntary Revocation Order to avoid the Department's continued investigation of his insurance related practices and activities. He is no longer able to effect insurance transactions in the state, without violating Illinois Insurance Code and facing felony charges."
