On September 6, 2019, Southwestern Eye Center entered into a Consent Agreement with the Arizona State Board of Dispensing Opticians. The Agreement settles allegations that the business was operating without a valid license or an establishment licence as required under Arizona Statutes. Under terms of the order, the business agrees to pay a civil penalty in the amount of $1,500. The Assurance was for settlement purposes only and should not be considered as an admission of guilt or finding of violation of the law.

The agreement settles allegations that the Sierra Vista location did not have a licensed dispensing optician employed full-time on-site. Under terms of the order, the business must pay a civil penalty totaling $1,250.
