On April 8, 2020, Alexander Villares, MD entered in to an Order for Letter of Reprimand and Probation with the Arizona Medical Board. The order settles allegations that Alexander Villares failed or refused to maintain adequate records on a patient and committing any conduct or practice that is or might be harmful or dangerous to the health of the patient or the public. Under terms of the order, Alexander Villares agreed to complete a minimum of 10 hours of Board staff pre-approved Category 1 non-disciplinary CME in an intensive, in person course regarding medical record keeping. Alexander Villares will be on probation for 12 months from the effective date of April 8th, 2020.
